Pre-Return To Work Form With Microsoft Forms
As part of our measures to return to the office safely, we needed our staff to complete and return the Pre-return to Work Form as set out in the Government’s Return to Work Safely Protocol. Our COVID-19 Response Team developed an online solution for our staff to complete this form and to collate the results to reduce the administrative burden of applying the measures set out in this protocol. We would like to share the template with you to assist you to apply these measures as painlessly as possible. Helpfully, the Health and Safety Authority (HSA) have confirmed to us that our form meets the requirements of the protocol.
Our solution uses Microsoft Forms and is simple to setup and use. You should not need any specialist assistance. Once an employee fills out the form and confirms the information to be accurate and correct, it is returned to you via Microsoft Forms. Microsoft Forms will provide the detailed responses by individual together with a dashboard showing all the results. This solution is GDPR compliant as the information submitted is stored within your own organisation’s Office 365 environment.
